Your Complete BBL Recovery Cheat Sheet

How long is BBL recovery? Recovery after BBL takes about 6 to 8 weeks. Most swelling is gone by week 6, and final results show around 3 to 6 months. You’ll need to avoid sitting directly on your butt for at least 6 weeks and follow post-op instructions closely to protect the fat transfer and speed up healing.

This guide covers what to expect during recovery, how to sit and sleep, what not to do, and how to maintain your results long-term.

What to Expect Immediately After BBL Surgery

Right after your surgery, you’re going to feel sore, swollen, and probably a little out of it. That’s totally normal. You’ll be in a compression garment, and you might notice some drainage from the liposuction site. That’s expected too.

You’ll need someone to take you home and stay with you for at least the first day. Moving around (slowly) is important to keep your blood flowing, but no sitting or lying on your butt just yet.

What’s the First Week Like After BBL?

The first week is usually the toughest. Expect soreness, swelling, and a lot of downtime as your body starts healing and adjusting to the fat transfer.

You’ll leave Dr. Rojas’s office in a compression garment, and you’ll be wearing it 24/7 (except when you shower). It helps reduce swelling and keeps everything in place as your body heals. Stick to loose, comfy clothes. Avoid wearing anything tight around your waist or hips.

YOU CANNOT SIT OR LIE ON YOUR BUTT AT ALL. This is very important. Sitting puts pressure on the newly transferred fat cells and can affect your results. Use a BBL pillow if you have to sit, but try to avoid it altogether. Sleep on your stomach or side with pillows to keep you in place.

Expect fluid drainage from the liposuction incisions for the first few days. It might look a little messy, but it’s normal. Bruising and swelling usually peak around day 3 or 4, then slowly start to fade. Short walks around the house every few hours are important to keep your blood flowing and reduce the risk of blood clots. But take it slow. No lifting, stretching, or workouts.

You’ll also need help. Someone should stay with you for at least the first 24–48 hours to help you move around, prep meals, and make sure you’re okay. Having a post-op kit ready (like wipes, snacks, extra pillows, meds, compression socks) makes a huge difference.

What Not to Do During BBL Recovery

To protect your results, there are a few things you’ll want to avoid, especially during the first few weeks:

  1. Don’t sit directly on your butt. Always use a BBL pillow or avoid sitting altogether until your surgeon gives the green light.
  1. Don’t sleep on your back. Stick to stomach or side sleeping for at least 6 weeks.
  1. Don’t skip your compression garment. It helps reduce swelling and keeps everything in place while you heal.
  1. Don’t smoke or drink alcohol. Both can slow healing and reduce fat survival.
  1. Don’t work out too soon. Wait until you’re cleared, especially for glute or high-impact exercises.
  1. Don’t ignore signs of complications. Sharp pain, fever, or unusual drainage? Call your surgeon right away.

When and How to Sit After a BBL Procedure

Wait at least 6 weeks before sitting directly on your butt. That pressure can damage the fat cells and affect your results.

If you have to sit (for a car ride, a quick meal, etc.), use a BBL pillow or sit on a cushioned surface with your weight on your thighs, not your butt. Sit upright, with your back straight, and avoid slouching or leaning back into the transferred area.

By week 6, most patients can sit normally again without a pillow. But even then, ease into it. Start with short periods and build up gradually. We also recommend keeping your BBL pillow in the car and at work. If you’re sitting for long periods, set a timer to stand and stretch every 30–45 minutes.

How to Sleep After BBL Surgery

You can’t sleep on your back for at least 4 to 6 weeks after your BBL. Putting pressure on the fat grafts while they’re healing can affect your results.

The best way to sleep is on your stomach or on your side with pillows to keep you from rolling over. It takes some getting used to, especially if you’re normally a back sleeper.

Here’s what helps:

  • Body pillows or rolled-up towels behind your back to stop you from flipping in your sleep
  • Wedge pillows to support your upper body if you’re a stomach sleeper
  • Pillow between your legs if you’re sleeping on your side—for comfort and support

It won’t be the most restful sleep at first, but your body will adjust. Try to create a routine and keep your setup consistent. After 6 weeks (or when your surgeon gives the okay), you can start easing back into your regular sleep position.

1 Month After BBL Surgery

By the 1-month mark, most of your swelling will go down, and you’ll start seeing the shape of your new curves. You may be cleared to sit with a BBL pillow for longer periods, but you’ll still need to avoid full pressure on your butt. 

Light exercise, like walking or gentle cardio, will likely be okay. But anything high-impact or glute-focused will still need to wait. You’ll still be wearing your compression garment daily and continuing lymphatic massages if your surgeon recommends them. 

Expect some fat absorption and minor changes. Tour results won’t be final yet, but you’ll be close.

2 Months After BBL Surgery

At 2 months post-op, you’re in the clear for most normal activities. The fat that’s going to survive has settled, and your results are starting to look more defined and natural.

You can usually sit, sleep, and work out like normal again. Just keep listening to your body and your surgeon’s advice. If you haven’t already, you can ease back into strength training, including glute exercises like squats and lunges.

Swelling should be minimal at this point, but small changes may still happen as your body continues to heal. You might notice your shape improving week by week. Most patients stop wearing their compression garment by now, unless your surgeon recommends longer use.

3 Months After BBL Surgery

By 3 months, your results should be close to final. Swelling will be gone, your shape will be sculpted, and your body will feel back to normal. You’ll be cleared for all activities, including sitting, sleeping, and working out without restrictions.

Now’s the time to shift from recovery to maintenance. You’ll want to keep your weight stable, stay active, and focus on glute-building workouts to protect and enhance your results. Don’t skip your follow-up if you haven’t had it yet.

How Painful Is BBL Recovery?

You’ll be uncomfortable, especially during the first few weeks, but it’s not unbearable. Most of the pain comes from the liposuction, not the fat transfer. You’ll feel sore, swollen, and tight. It’ll feel like deep bruising or muscle soreness. 

Pain meds will help, and most of the discomfort eases up by the end of week one. Tenderness may linger for a couple of weeks, but sharp pain isn’t typical. If you feel anything severe or unusual, call your surgeon.
